How to request an Info kit

Physical Infokit

1. Procedures for requesting a Free Physical info kit form

1. Navigate to the website https://cr.uat.icuboid.com

 2. Click on the free info kit Icon on the home page

  3. Click on the info kit by mail option

  4. Fill in all the fields on the form and then click on the  submit button

5. We will get a confirmation message after submitting the physical info kit  

Digital Infokit

1. Procedures for requesting a free Digital info kit form

1. Navigate to the website https://cr.uat.icuboid.com

2. Click on the free info kit Icon on the home page ( Please refer to img1)

3. Click on the Info kit download option ( Please refer to img2)

4. Fill in all the fields on the form then click on the submit button

5. We will get a confirmation message after submitting the digital info kit  

6. The customer will receive an email containing the free digital info kit.
